				Overview:

The Corporate Health Wellness Center Nurse facilitates the identification, assessment, and management of corporate client wellness program participants. Care scope may include employees, dependents, retirees, and/or students based on health risk factors and episodic care needs.  Responsibilities include completing disease specific assessments of risk and developing care plans that promote healthy lifestyles and self-management of risk levels and disease states.  This position may provide one-on-one coaching for corporate client employees, dependents, retirees, and/or students and includes, but is not limited to, empowering and supporting behavior change aimed at reducing health risks, improving health status, and improving quality of life for participants.

Qualifications:

* Education:  Bachelor's degree preferred.
* Experience:  Minimum two years of clinical experience in a related field required.
* Licensure/Certification/Registration:  RN currently licensed in appropriate state required.  AHA Heartcode BLS required.
* Additional Skills/Requirements required:  Ability to successfully complete generic and department specific skills validation and competency testing.  Ability to work independently without supervision and self-manage.  Evaluate statistical information and make sound judgements.  High energy with strong verbal and written communication skills.  Ability to effectively communicate with a wide range of individuals and constituencies amoung a diverse population.  Have a passion for health improvement and be able to communicate case sensitive material to participants.  Ability to develop, plan and implement short and long range goals.  Knowledge of reading, understanding and evaluating participant's labs, charts, history and medication records.
