Overview: This job is located in Peru,IL Monday-Friday 8:00-4:30pm POSITION SUMMARY: The Hospice Bereavement Coordinator is responsible for coordinating and directing bereavement services to patients and families. The Bereavement Coordinator works closely with other members of the hospice team in assisting with holistic care of patients and families. Provides individual and group counseling, facilities grief support group, and coordinates memorial services. The Bereavement Coordinator shall demonstrates knowledge of the principles of growth and development as well as the emotional and psychosocial needs from all age groups. Responsible for making visits in various settings, and working evenings or weekends when assigned. Qualifications: REQUIRED QUALIFICATIONS: * High School Diploma and a minimum of 1 year advanced education PREFERRED QUALIFICATIONS: * Bachelor Degree or experience in the practice of counseling with a variety of age groups or * Previous experience in bereavement, clinical counseling or social service work Licensure/Certifications Other skills or knowledge * Knowledge of state, federal and JCAHO regulations * Computer skills * Knowledge of EPIC preferred * CPR encouraged but not required * Ability to work independently and execute own work load OSF HealthCare is an Equal Opportunity Employer.
