The LVN performs patient care management services that support the established plan of care as directed by the other licensed staff within the team (department). This includes assisting in relaying instructions from the physician to a patient or authorized person, and collecting patient data, documenting patient concerns, patient messages, and any instructions or education provided within the care management operational platform. Providing leadership to support staff.
Qualifications:
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
State issued license as a Licensed Vocational or Practical Nurse
* Graduate of an accredited school of nursing: Licensed Vocational or Practical Nursing.
* 2 years related experience or advanced degree required, i.e. bachelors degree in the healthcare field
* 3 to 5 years related experience preferred
Physical Demand Level: Light
This remote LPN needs to be able to travel between both our corporate location a few times a month to handle some of the job duties. We do require the LPN to live in Colorado.
