The purpose of Dignity Health Management Services Organization (Dignity Health MSO) is to build a system-wide integrated physician-centric, full-service management service organization structure. Responsibilities:

As the Continuing Care Assistant, you will primarily assist both patients and members of the department and healthcare team, but also may work with colleagues across the care coordination continuum.

Every day, you will collaborate with the team members to achieve member goals, telephonically assist members, and perform other administrative duties.

To be successful, you will demonstrate excellent customer service and communication skills, attention to detail and prioritization, and a patient demeanor, contributing to a supportive and respectful environment for those entrusted to our care.

- Listen to understand patient needs, and connect them to appropriate resources both inside and outside of the department / team.
- Schedule appointments, assist with referrals, answer questions, and other similar duties as assigned, escalate issues as needed.
- Receive patient requests and refer to appropriate members of the team, community, or within the organization. Identify problems with healthcare access and utilization, along with providing alternatives if available, escalate as indicated/directed. Act as liaison for members and families to physicians, staff, community resources, and others. Efficiently communicate and collaborate with a multidisciplinary team across the care continuum in order to meet patient needs.
- Participate in departmental and organizational committees, meetings and projects as directed.
- Clearly communicate the purposes and services of assigned programs to the patient and caregivers.
- Manage assigned caseloads and routinely document interactions as per company policy, escalating issues as directed.

Qualifications:

Minimum Qualifications:

- 2+ years experience in a professional and/or healthcare environment.
- High school diploma or GED.

Preferred Qualifications:

- Certified Medical Assistant (MA) preferred.
- Knowledge and ability to work with various electronic medical records and platforms preferred.

- Spreadsheet skills (Excel/Google Sheets) preferred.

- Knowledge of medical terminology preferred.
