Overview: Responsibilities: Plans, organizes, coordinates, and implements comprehensive discharge planning and social work services to patients who have significant medical, financial, legal, and/or social barriers to timely discharge from the acute care setting. This position works closely with legal services, risk management, and medical directors in order to provide services which meet IH's standards of quality, efficiency and desired outcomes. Qualifications: Minimum Qualifications: BSW 10 years' experience in an acute care medical setting including discharge planning. Working knowledge of management principles. Extensive knowledge of local community/agency resources and ability to network effectively with post-acute providers and establish new relationships. Proven leadership skills and ability to work effectively with physicians, nurses, and ancillary departments. Licensure/Registration/Certification: Current LBSW Desired Qualifications: MSW or Master's degree in a related field Desired Licensure/Registration/Certification: Current LGSW or LCSW
