Job Summary:

The Community Hub Supervisor manages program administration and staff supervision for the YMCA's Community Hub contract, which is administered by King County's Accountable Community of Health known as HealthierHere. The Community Hub seeks to fulfill the Health Related Social Needs (HRSN's) of King County residents that are furthest from health equity by assigning them Navigators who provide the information services and resource navigation they need to thrive. This work is mostly virtual for YMCA Community Hub Navigators, with limited in-person client meetings as necessary. The Hub Supervisor will include a combination of office, remote, and field work.
 
As the Supervisor of this program, you will: oversee Community Hub Navigator staff in rendering high-quality case management services, seek out outreach opportunities to connect human services providers and potential clients into the Hub, coordinate training opportunities for staff, formalize processes and procedures to ensure efficient program flow, and fulfill other grant deliverables as assigned. Additionally, the Community Hub Supervisor will strengthen the YMCA Health Integration team's commitment to advancing health equity by leading efforts screen for and address Social Determinants of Health of participants across our Chronic Disease Prevention programs. This may include developing best practices for the Y Healthy Living department to use in addressing Social Determinant of Health (SDOH) needs of the individuals we serve, including processes for identifying clients' unmet health and social needs and connecting them to necessary internal and external resources; building strong relationships with Y program participants to provide health navigation services; and supporting Y Health Coaches in their roles of connecting clients to health and social services  

Hiring Range: $32.00 - $38.00/hour

Responsibilities:

* Make initial contact with each Community Hub client within 24 hours of referral receipt.
* Complete client intake process with complete documentation, and triage client assignments to most appropriate YMCA Community Hub Navigator.
* Supervise Community Hub Navigator team to ensure clients receive high quality care through effective resource connection, consistent follow-up, and positive relationship building. 
* Use the Connect 2 Coordinator electronic records platform to ensure Navigator team is properly documenting each resource referral, client communication summary, demographic input, and other deliverables with professionalism. 
* Provide guidance and intervention to Navigator staff when they are assisting clients who are escalated, in crises, and/or seeking help with particularly complex service needs. 
* Create and continually revise the YMCA Community Hub team's programmatic standards, SOP's, and team goals, with limited oversight. 
* Ensure program outcome requirements are met and services are rendered appropriately by understanding contract/grant terms and conditions. 
* Utilize best-practice philosophies in the field, including trauma-informed care, motivational interviewing, and harm reduction to guide Navigators in service delivery.  
* Develop trainings, workshops, process guides, and other teaching tools to educate YMCA staff on the value of the Community Hub, the Community Hub's referral process, and on screening tools for Social Determinants of Health. 
* Team effectively and collaboratively with other YMCA staff and/or community partners to plan and facilitate outreach activities, including presenting workshops and staffing special events. 
* Build and maintain collaborative relationships with funding liaisons, other agencies in the Community Hub network, and external service providers. 
* Creates screening tools and training plans to improve the Healthy Living Team's ability to identify and address Social Determinants of Health (SDOH). 
* Other duties as assigned.

While performing the duties of this job, the employee is regularly required to talk or hear.  The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.  This position requires the ability to occasionally lift office products and supplies, up to 20 pounds.

POSITION TYPE/EXPECTED HOURS OF WORK

This is a full-time position. The support center is staffed 7 days/week and operational hours match branch hours. Evening and weekend work may be required as job duties demand.

TRAVEL

Travel is primarily local, although some out-of-the-area and overnight travel may be expected.

Qualifications:

* 21 years of age or older. 
* Master's degree or equivalent experience in social work, social services, or closely related field. 
* Two or more years of supervising staff in a human services or health care environment. 
* Intermediate computer skills, experience with Microsoft Office suite, and experience using electronic databases to record client interactions.
* Strong advocate, with ability to effectively communicate the needs of clients, case managers, and the Community Hub program to funders and to YMCA leaders. 
* Knowledge of and previous experience with diverse populations (language, culture, race, physical ability, sexual orientation, etc.)  
* Experience working collaboratively with multiple stakeholders, such as clients, supervisees, community resources, government agencies, funders, and coworkers.
* Ability to visit sites and community organizations throughout King County. 

Preferred Qualifications:

* Comfort and experience directly supporting clients and/or their case managers in the midst of crisis and traumatic events, including but not limited to: domestic violence, substance use, child protective services-involvement, presentation of severe mental health challenges, and suicidality.
* Knowledge of resources and legal guidelines for people experiencing homelessness, facing eviction, and in general housing crisis in King County.
* Experience with supervising staff in a remote working environment that requires significant autonomy.
* Ability to build relationships with referral partners across King County through in-person visits and virtual meetings.
* Knowledge of and previous experience with diverse populations (language, culture, race, physical ability, sexual orientation, etc.). 
* Ability to speak any language in addition to English may be helpful.
* Knowledge of and experience with YMCA Healthy Living Programs preferred. 
* Proven history of successfully delivering projects on time and within budget.
