Presbyterian is now hiring a Community Health Worker - Level 1 for Northern New Mexico. The role is located in an outpatient clinic setting, and will be providing support and navigation to patients during pregnancy and postpartum. This position will provide patients with basic health education, referrals to community-based organizations and community health programs and access to care navigation. Responsibilities: * Participate in the planning and implementing, the plan of care as developed by Care Manager * Report changes in patient condition as reported by patient to Care Manager and/or Primary Care Provider as appropriate. Support patient and family teaching. * Maintain a safe environment for patients, families and visitors. * Keep current with community health care worker standards, PHS philosophy, and PHS programs in order to maintain quality. * Maintains education development through participation in education programs current literature, inservice meetings and workshops * Performs other functions as required. * Interacts effectively with customers, providing instructions and information, both written and verbal as appropriate * Participates in local, regional or state community health care worker events/meetings Qualifications: * High school plus specialized, short-term training in clinical, medical skills. * Must be able to speak language of the population served. Education Essential: High School Diploma or GED
