Responsibilities:

Position Summary:

Assist individuals and communities to adopt healthy behaviors. Conduct outreach for medical personnel or health organizations to implement programs in the community that promote maintain and improve individual and community health. May provide information on available resources provide social support and informal counseling advocate for individuals and community health needs and provide services such as blood pressure screening. May collect data to help identify community health needs.

Qualifications:

Minimum:

* Experience working in the community.
* High School Diploma/GED
* Ability to work independently set priorities and manage work within timeframes read and analyze technical material communicate effectively verbally and in writing positive mental attitude. Strong customer service orientation and the ability to handle highly confidential information essential. Must be computer literate.
* Will complete Community Health Worker (CHW) or Promotoras training within a year of hire.

Preferred Qualifications:

* Teaching
* Bachelors degree
* Bilingual (English/Spanish)
* Completed Community Health Worker (CHW) or Promotoras training.
