Clinical Service Navigator (RN)
Location: Winston-Salem, North Carolina
Department: Neuro & Sleep Clinic
Employment Type: Part Time, 32 hours per week. Monday-Thursday 8am-5pm
Position Overview
* Assist clinic with rooming of patients.
* Triage patient calls.
* Explain all processes to patients in plain language and utilize teach back to ensure understanding.
* Provide exceptional patient care, demonstrating empathy and professionalism.
* Collaborate with a multidisciplinary team to deliver high-quality healthcare services.
* Be active participant in stroke support group.
* Monitor schedule for appropriateness.
* Call patients post discharge from hospital to ensure continuity of care.

Qualifications:
* Education:
* High School or GED, required.
* 4 Year / Bachelors Degree, preferred.
* Experience:
* Three to five years of nursing experience, required.
* Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.
* Licensure/Certification:
* RN license in Appropriate State, REQUIRED
* Additional Skills/Requirements (required):
* Ability to successfully complete generic and department-specific skills validation and competency testing.
