Overview:
Schedule: Monday - Friday 8:00 AM - 5:00 PM; Full-Time
Department: Care Navigation - supporting heart failure patients
Location: Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte.
This role is ideal for an experienced RN looking to serve as the primary liaison between the patient and his/her medical team and be a patient advocate.

Responsibilities:
* Support heart failure patients on any unit at PMC
* Find resources for patients including food, medicine, and clinics
* Work to discover what is causing the patient to return to the hospital
* Review medication and diet adherance issues with the patient
* Provide some educaiton on the patient's diagnosis
* Partner with case management and palliative care teams
* Work towards goal of reducing readmission and ER visits
Qualifications:
* Education: 4 Year / Bachelors Degree, required.
* Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.
* Licensure/Certification: RN, required.
* Additional Skills/Requirements (required): Ability to successfully complete generic and department-specific skills validation and competency testing.
