AMBULATORY CARE Clinical Pharmacist New Hanover/Wilmington, NC

Novant Health is seeking a Clinical Pharmacist, to join our ambulatory pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

Preferred Experience/Credentials

  Providing patient care as a Clinical Pharmacist Practitioner (CPP) or equivalent

  Direct patient care experience of 1-2 years in primary care setting with emphasis on diabetes and endocrine disorders, HIV, and other primary care conditions.

  PGY2 Ambulatory residency or equivalent experience

  Current BCPS or BCACP (board certification will be required post-employment for credentialing)

Responsibilities:

Essential functions include:

  This embedded position will staff an endocrinology clinic four days per week and a Ryan White HIV clinic 1 day per week.

  Partnership with NHMG providers and clinical staff to provide remarkable patient care.

  Medication therapy management and patient counseling.

  Promotes high quality, evidence-based, cost-effective pharmaceutical care.

  Precepting of residents (PGY1/PGY2) and pharmacy students.

  Participation in pharmacy and NHMG committees and shared governance.

  Have a clear understanding of quality improvement principles.

  Contributes to process improvement projects.

  Supports the educational and professional development of the department and system.

  Ability to communicate effectively both orally and in writing.

  Demonstrating proficiency with equipment and software

Qualifications:

* Education: Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. PharmD, preferred.
* Experience:
* Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy. Oncology experience preferred for oncology roles, required.
* ASHP accredited PGY2 Infectious disease residency OR 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required.

* Licensure/Certification:
* Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
* Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role, required.

* Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
