				Overview:

This position will be supporting pharmacy services at OSF Divine Mercy Continued Care in Peoria, IL

8:00am-4:30pm Monday-Friday. 8:00am-12:00pm on weekends and holidays. Rotating weekend/holiday schedule. Rotating on-call schedule when the pharmacy is closed

POSITION SUMMARY: The Clinical Pharmacist reviews prescriptions, obtains clarifications, analyzes drug therapy and regularly consults with medical staff, nursing personnel and others in order to coordinate delivery of high quality pharmaceutical services to OSF HealthCare patients. In addition, the clinical staff pharmacist is responsible for compounding and dispensing medications and other pharmaceutical supplies according to established procedures and professional pharmacy practice. Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served.

Qualifications:

REQUIRED QUALIFICATIONS: Licenses/Certifications:

PHRM-Pharmacist 

American Heart Association (AHA) BLS required within 90 days of hire.

 PREFERRED QUALIFICATIONS: Education: Doctorate of Pharmacy (PharmD) or Pharmacy Practice PGY1

Experience: 2 years of hospital pharmacy experience serving a variety of patient types. Experience with IV compounding/USP797 & USP800.

 Licensure/Certifications: Board Certification through Board of Pharmacy Specialties

 Other requirements/information: Interpersonal skills necessary in order to communicate effectively with physicians, patients, and various other OSF HealthCare personnel when explaining medication use and gathering and exchanging other medication information. Analytical skills necessary in order to determine drug appropriateness and evaluate drug interactions. Demonstrated skills in independent problem solving, decision‑making abilities, and prioritization/delegation Demonstrated knowledge of the principles of growth and development as well as the physical, emotional, and psychosocial needs of neonates, pediatrics, adults, and geriatrics. Excellent computer skills and the ability to process detailed information in an accurate manner. Competency in the use of an electronic medical record system.
