Responsibilities:
* Collaborate with physicians and providers to care for clinic patients
* Work closely with providers to develop appropriate treatment plans
* Manage ongoing medication dosing and patient monitoring
* Follow institutional medication management goals (e.g., medication evaluations and reconciliation)
* Educate staff on updated guidelines and best practices for this patient population
* Help manage medication-related toxicities and adverse effects for clinic patients
*Must be a registered Pharmacist and licensed CPP in the state of NC within 1 year of hire
Qualifications:
* Education: 4 Year/Bachelors Degree, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification. Doctorate of Pharmacy, required. PharmD, preferred.
* Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
* Licensure/Certification:
* Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
* Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
* Additional skills required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
