Clinical Pharmacist (Ambulatory Care)
Full-time / Monday - Friday, 8am-5pm
Hybrid Position, supporting Winston-Salem
Novant Health is seeking a Clinical Pharmacist, to join our ambulatory pharmacy team, to deliver optimal medication management across all dimensions of care, every time.
Preferred Experience/Credentials
* Providing patient care as a Clinical Pharmacist Practitioner (CPP) or equivalent
* Direct patient care experience of 1-2 years in primary care setting with emphasis on diabetes, anticoagulation, lipids, HTN, obesity, osteoporosis patient management
* PGY2 Ambulatory residency or equivalent experience
* Current BCPS or BCACP (board certification will be required post-employment for credentialing)
* Proficient bilingual skills in Spanish
Essential functions include:
* Partnership with NHMG providers and clinical staff to provide remarkable patient care.
* Medication therapy management and patient counseling.
* Promotes high quality, evidence-based, cost-effective pharmaceutical care.
* Precepting of residents (PGY1/PGY2) and students.
* Participation in pharmacy and NHMG committees and shared governance.
* Have a clear understanding of quality improvement principles.
* Contributes to process improvement projects.
* Supports the educational and professional development of the department and system.
* Ability to communicate effectively both orally and in writing.
* Demonstrating proficiency with equipment and software.
Remote opportunity within our approved hiring footprint. Will need to come on-site as needed.
* Onboarding will require in person relationship building with clinics
* Additional onsite presence may be required to maintain relationships and build referral volume

Qualifications:
* Education: Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. Doctorate of Pharmacy, preferred.
* Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
* Licensure/Certification:
* Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
* Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
* Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
