Responsibilities:

The specialty medication prescribing service will drive specialty medication management, working with specialty pharmacy operations and ambulatory clinic pharmacists in specialty care to support multiple locations.

The centralized service, consisting of pharmacists and a pharmacy technician, will focus on optimizing medication strategies and enhancing patient outcomes across our health system.

Successful candidates will need to travel to on-site clinic locations.

Qualifications:

Required Education and Experience

* Graduation from an accredited school of pharmacy

Preferred Education and Experience

* Pharm.D. with: completion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities, or Completion of a PGY2 Ambulatory Care Pharmacy residency, or Completion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities

Required Licensure and Certifications

* Active Pharmacist License in good standing in the state(s) in which the employee works. PHARM:WA

Required Minimum Knowledge, Skills, Abilities and Training

* Current knowledge of evidence based clinical pharmacy practice and pharmacotherapies
* Knowledge of Health Plans such as Medicare, Medicaid, and commercial plans
* Demonstrates knowledge and skills necessary to provide care appropriate to all age groups of patients served and infection control, safety and risk management standards as relates to the ambulatory pharmacy setting.
