The Clinical Outcomes Specialist acts as a subject matter expert for key EMR clinical workflows, ensuring all clinical staff are competent and efficient in their use. This clinical resource will partner with the Ambulatory Quality and Patient Safety (AQPS) Team. This role is also responsible for actively managing and closing gaps in care for the practice patient population, navigating a patient's Social Determinants of Health (SDOH) opportunities through existing organizational relationships, and coordinating follow-up appointments for preventive screenings and ongoing chronic disease management. This position is vital to our quality improvement initiatives and directly impacts patient outcomes. Responsibilities: A day in the life of a Clinical Outcomes Specialist at Hackensack Meridian Health includes: * EMR and Clinical Workflow Management: This resource will work closely with their assigned AQPS Practice Facilitator and be expected to function as a superuser in the clinical location. Competency in key workflows and comfort level in independently providing EPIC EMR clinical elbow support, is expected within 6 months of hire. Competency will be tested using an existing rubric-based workflow, a transparent and standardized process that defines performance expectations and assesses proficiency in specific skills. Act as the clinic's primary resource and subject matter expert for the core group of key quality EMR clinical workflows. This role will ensure clinical competency of key quality EMR clinical workflows for the practice team members. Develop, implement, and maintain local training materials to ensure all clinical team members are proficient in using the EMR system. Conduct regular/ongoing competency assessments and provide one-on-one or group training to address any skill gaps. Work collaboratively with clinic leadership and IT to troubleshoot EMR issues and optimize workflows for improved efficiency and patient safety. Stay up-to-date on EMR updates and new features, communicating changes to the clinical staff. * Population Health Management and Quality Improvement: Proactively manage and close gaps in care for targeted populations, focusing on key initiatives such as (not limited to): Breast cancer screening outreach Colorectal cancer screening outreach Transition of care management EMR workflow CMA, front desk, and provider Annual wellness visit scheduling and pre-planning. EMR workflow CMA, front desk, and provider Three-month chronic disease follow-up appointment scheduling Previsit planning daily Lab and image tracking Escalate clinical concerns to Providers and Manager (as needed). Utilize EMR reporting and dashboards to identify patients needing intervention and track progress on quality metrics. Leverage local sprints and action planning to improve quality. Support the clinic's quality improvement initiatives by providing data and feedback on workflow performance and care gap closure. 3. Patient Navigation and Community Resource Coordination: Serve as a navigator for the patient population, connecting them with essential community resources related to SDOH, leveraging existing community partners for housing, food security, transportation, and other needs. Educate patients on how to access and utilize these resources effectively. Proficient documentation of all patient navigation activities and outcomes in the EMR to ensure a complete and coordinated plan of care. 4. Regional travel required, use of personal vehicle with mileage reimbursement (consistent with company policy) - driver's license required. * Other duties and/or projects as assigned. * Adheres to HMH Organizational competencies and standards of behavior. Qualifications: Education, Knowledge, Skills and Abilities Required: * High School diploma, general equivalency diploma (GED), and/or GED equivalent programs. * Graduate of an accredited Medical Assistant program or LPN program. * Minimum of 3-5 years of experience as a CMA, preferably in a hospital or ambulatory practice setting. * Excellent written and verbal communication skills. * Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms. Education, Knowledge, Skills and Abilities Preferred: * Experience with NCQA PCMH standards and the accreditation process. * Experience with HEDIS, MIPS, and other value-based care quality measures. Licenses and Certifications Required: * Licensed Practical Nurse or Certified Medical Assistant. * Valid Driver's License from a USA state. Licenses and Certifications Preferred: * Patient-Centered Medical Home Certified Content Expert.
