Responsibilities:
The Clinical Care Manager serves as part of a self-directed work team to provide care management and therapeutic connection for clients with disabling HIV/AIDS in Outpatient Services and clients in the Residential Care Program. This job also manages the assigned caseload, provides support to the care team, coordinates client services, and creates and implements conuseling treatment plans.

Our services include a 35 bed inpatient program that focuses primarily on individuals who are experiencing end of life due to HIV, ALS, Huntingdon's Disease or cancer. Our outpatient program serves approximately 360 individuals who have HIV/AIDS complicated by homelessness, mental illness, justice system involvement and/or chemical dependency. This particular position will support our outpatient services. We believe in putting the patient and their needs first, so every Bailey-Boushay clinician learns to do every job. Days are never the same and could include a mix of Nursing, serving lunch, helping a client with a shower, etc.
Candidates should possess the willingness to work in a nontraditional position among patients in a milieu setting vs a job that is anchored at a desk. Since we are a 7 day a week operation, staff share weekend coverage on a rotational basis. Staff work together to produce a staffing schedule to meet the needs of the team. Qualifications:
Bachelor's degree Social Work, counseling, or a related field, or equivalent required. Registration as an Agency Affiliated Counselor by the state of Washington required. Also required are: excellent interpersonal skills; excellent communication skills; excellent case management and time management skills; skill in using PC and software (e.g., MS Office); ability to respond appropriately in stressful situations.Preferred: Three years' experience in a counseling capacity, including experience serving as a part of a multi-disciplinary team serving minority populations, and populations with issues of chemical dependency and mental health. Position does not involve chemical dependency counseling. Working knowledge of grief, bereavement, dementia, and end of life care a plus. Fluency in Spanish strongly preferred.
