Responsibilities:

**Up to $5000 Sign On Bonus Offered**

Paid out in FULL after 30 days of employment!

Location: Immanuel Campus off of 72nd & Center

Schedule: Monday-Friday 8a-5p, NO weekends, NO holidays & NO call! 

Are you a Registered Nurse (RN) who believes healthcare should be proactive and personalized? We're looking for a passionate RN to join our vibrant clinic team working as a Wellness Clinic RN, dedicated to serving our valued Medicare patients. At our clinic, we foster a supportive culture guided by our core principles of patient-first care, community, and having fun while making a real difference. This position is also a dual role! Not only will you be helping our value medicare patients you will also be helping patients transition after getting out of the hospital! As the person who is a Care Navigator, you are the vital link in the patient's journey, the person who connects the dots and ensures a smooth "handoff" as they recover. You'll work with a diverse team of physicians, nurses, and social workers to coordinate care, educate patients and families, and ultimately, prevent unnecessary hospital readmissions. This role is perfect for a highly organized and empathetic professional who thrives on variety and is ready to champion patient wellness.
Qualifications:

Minimum Qualifications:

* Current Registered Nurse (RN) license that allows you to practice in our state
* Graduate of an accredited nursing program
* Basic Life Support (BLS) from the American Heart Association required
