Overview: Responsible for providing education to high-risk pregnant women diagnosed with type one, type two, and gestational diabetes. To include Insulin Teaching, Nutrition Counseling, Blood Glucose Guidance, Glucose Meter, and Insulin Equipment. Responsibilities: * Work as a team with patients/families providing education and guidance related to diabetes and self-management skills recognizing warning signs and symptoms as it relates to maternal and fetal health consistent with the Standards of Care of the American Diabetes Association (ADA) and ACOG guidelines. * Develop a personalized treatment plan for a patient's blood glucose levels by monitoring/interpreting the weekly logs and nutrition journal working with the physician to adjust diet and medication as needed for optimal control during pregnancy. * Teach patients what to do if blood glucose is too high or low and how to mitigate these risks. * Provides teaching of insulin pumps, and glucose monitoring systems using demonstration and repetition for patient competence. * Demonstrates continued learning and knowledge of diabetes technology, devices, and nutrition counseling. * Equip patients with the education needed for proper nutrition and carb counting using teach-back method for clear understanding. * Emphasize the following areas: Healthy eating, activity, monitoring, medication, problem solving, healthy coping, and reducing risks. * Is a resource to diabetic patients and their family providing guidance or referral as needed. * Is a resource to OB providers, nurses and other staff related to Antepartum and Intrapartum management of insulin and nutrition. * Works with Perinatal physicians and ARNP to develop appropriate care plan for patients and participates in weekly patient care review. * Builds and establishes positive working relationships with hospital staff, community agencies, the public and other members of the interdisciplinary team. * Informed of resources in the community for patients that need financial assistance or are at greater risk of food insecurity. * Keep track of clinical statistics such as volume, cases, types of patients seen etc. * Be aware of what is happening in the department and organization by attending department meetings, reading emails, and regularly checking information on the Hub. * Maintains clinical competency using the nursing process to provide quality care. * Maintains certification in diabetes education and attends Sweet Success or other diabetic education conferences related to pregnancy. * Always striving to exceed patient's expectations. * Demonstrates the knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately. * Integrates the UnityPoint Health-Des Moines nursing expectations of performance in all aspects of care delivery and professional practice. * Continues professional development and involvement in professional activities that enhance nursing practice by sharing knowledge through networking and research. * Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines. * Demonstrates ability to meet business needs of department with regular, reliable attendance. * Employee maintains current licenses and/or certifications required for the position. * Practices and reflects knowledge of HIPAA, JCAHO, DNV, OSHA, CMS and other federal/state regulatory agencies guiding healthcare. * Completes all annual education and competency requirements within the designated time frame. * Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. * Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. * Takes appropriate action on concerns reported by department staff related to compliance. * This position may be assigned a specific mission to address in an emergency situation to assist in disaster response and recovery as part of the UPHDM Hospital Emergency Incident Command System. Situations of a critical nature may require an individual to perform multiple tasks until additional support can be obtained. * Demonstrates initiative to improve quality and customer service by striving to exceed expectations. Qualifications: EDUCATION: Graduate of an accredited school of nursing. Bachelore of Science in Nursing (BSN). Preferred Master of Science in Nursing (MSN). EXPERIENCE: Two or more years in Antepartum and Intrapartum nursing. One or more years as a Diabetes Educator with 400 hours or more of diabetes education. One or more years as a Perinatal Diabetes Educator. LICENSURE/CERTIFICATION: A state of Iowa nursing license to practice as an RN. Board Certified-Advanced Diabetes Management from American Association of Diabetes Educators (requires MSN) preferred. Certified Diabetes Educator (CDE) through (NCBDE). Licenses & Certifications must be maintained. Complete fifteen hours of relevant NCBDE-approved continuing education within two years of applying. Certification applicable to specialty area, EFM certification or Intermediate fetal monitoring, ACLS must be obtained within first year of hire. Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three months from date of hire. Valid driver's license when driving any vehicle for work-related reasons. KNOWLEDGE/SKILLS/ABILITIES:. Writes, reads, comprehends and speaks fluent English. Multicultural sensitivity. Communicate effectively (written and orally) with people of diverse professional, education and lifestyle backgrounds. Microsoft Office - basic computer skills. Customer/patient focused. Critical thinking skills using independent judgment in making decisions. Use of usual and customary equipment used to perform essential functions of the position.
