Responsibilities:

As our Clinic LPN at CHI St. Joseph's Health Primary Care Clinic, you'll help our patients by providing nursing care to our patients in our clinic under the direction of the physician or registered nurse. 

Every day you will work with our patients to evaluate their needs and provide physical or psychosocial care.  To be successful in this role, you must have a genuine compassion for your patients' well-being and understand the role you play in helping our our patients feel at ease. You also have the patience to explain the process, answer questions and update documentation. 

What you'll do...

* Conduct a focused assessment of an individual patient through the collection and comparison of data to normal findings and the patient's current health status.
* Report changes in assessment and responses to interventions in an ongoing manner to a registered nurse or the appropriate licensed health care provider for delegated or assigned tasks or activities.
* Participate in the development and modification of a plan of care.
* Implement interventions that are delegated, ordered, or prescribed by a licensed health care provider; determine and implement appropriate interventions within a nursing plan of care or when delegated or assigned by a registered nurse.
* Assign nursing activities or tasks to other LPNs; assign and monitor nursing tasks or activities to unlicensed assistive personnel.
* Providing safe and effective nursing care delivery.
* Promote a safe and therapeutic environment.
* Advocate for the best interests of individual patients.

Qualifications:

* Graduate of LPN education program
* LPN License in the State of Minnesota
* Basic Life Support (BLS) issued by American Heart Association.
