Under the supervision of the CHI Health at Home agency Director, the Hospice Chaplain is responsible for providing spiritual/bereavement care and counseling to Hospice patients, families, staff and volunteers. Essential Key Job Responsibilities 1. Documents assessment, planning and coordination of spiritual care of the patient/family: 2. Meets Spiritual care needs of patient and family 3. Coordinates agency Bereavement Services: 4. Facilitates and/or coordinates in-services, presentations and special events for the Hospice program: Qualifications: Bachelor's degree in Religious Studies or related field Current driver's license Current auto insurance BLS-CPR Preferred Home health experience
