Overview: 1st Shift:8:00am-4:00pm, Mon-Fri Primarily responsible for the provision of spiritual care services to meet the social, emotional, and spiritual needs of patients and families/caregivers, either directly or through coordination of care with other spiritual counselors. Responsibilities: * Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual needs. * Maintain record of spiritual care services and related activities for quality assurance, program development, and policies and procedures. * Document direct services and ongoing communication with community chaplains, spiritual counselors and hospice interdisciplinary team. * Provide consultation, education, and support to the interdisciplinary group on spiritual care. * Meets measures as determined and required for job productivity and performance improvement. * Provide bereavement follow up services as requested. * Assist in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. * Develop and maintain a resource group of clergy to whom specific aspects of spiritual care may be delegated. * Recruit community chaplains, spiritual counselors, and spiritual care volunteers adequately to meet patient/ family/caregiver needs. * Participates in quality assurance and performance improvement activities as assigned. Qualifications: Education: * Master's degree from an accredited school of theology, or related. * 1 Unit of CPE required License(s)/Certification(s): * Valid driver's license when driving any vehicle for work-related reasons. * Valid Mandatory Reporter course completion by state(s) requirement * CPR Certification. *Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs. The salary range listed does not include other forms of compensation which may include bonuses/incentive, differential pay, or other forms of compensation or benefits that may be applicable to this role.
