Overview: Primarily responsible for the provision of spiritual care services to meet the social, emotional, and spiritual needs of patients and families/caregivers, either directly or through coordination of care with other spiritual counselors. Responsibilities: * Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual needs. * Maintain record of spiritual care services and related activities for quality assurance, program development, and policies and procedures. * Document direct services and ongoing communication with community chaplains, spiritual counselors and hospice interdisciplinary team. * Attend Interdisciplinary Team meetings and complete all associated documentation in an accurate and timely manner. * Provide consultation, education, and support to the interdisciplinary group on spiritual care. * Meets measures as determined and required for job productivity and performance improvement. * Provide bereavement follow up services as requested. * Arrange for on-call availability of chaplaincy services. * Conduct or make arrangements for funeral or memorial services for patients as requested. * Assist in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. Qualifications: Education: * Graduate of accredited school of theology, or related Bachelor's degree. * 1 unit of CPE at time of hire. 2nd unit of CPE must be completed within 1 year of hire. Experience: * Ordination in a particular religious group and/or endorsed and in good standing from an ecclesiastical governing body Licensure: * Valid driver's license when driving any vehicle for work-related reasons. * Valid Mandatory Reporter course completion by state(s) requirement * CPR Certification. #UPaHomeNursing
