Schedule: PRN, 8 hour day shift (schedule is as needed)
Department: Case Management/Social Worker
Experience with Behavioral Health, homelessness, substance abuse, elderly care, Medicaid, assisted living, in facility placement is preferred. Must be familiar with working with physicians and nurse managers
We're seeking a MSW Case Manager to join our team where you will help to support patients and families during a time of need. You will collaborate with the individual, family, physician, and interdisciplinary teams to coordinate healthcare services to promote optimal clinical, financial, and satisfaction outcomes. Responsibilities:
* Provides counseling and crisis intervention to families and patients.
* Conducts psychosocial assessments.
* Serves as a resource to patients and families.
* Facilitates/ Assists patients and families in open discussions regarding advanced care planning.
* Being a patient advocate to ensure their needs are met and rights are protected.
Qualifications:
* Master's degree of Social Worker required.
