Case Manager

				Overview:

The Case Manager assesses plans, implements, coordinates, monitors and evaluates options and services to meet individual healthcare needs. Promotes quality and cost-effective outcomes.

Responsibilities:

* Interject objectivity, maximize efficiency and work collaboratively with patient, physician, family and in the development and execution of a healthcare plan.
* Assist client's self-direct care and make decisions to the degree possible.
* Appraise the need for intervention by gathering relevant data and conducting an objective evaluation.
* Evaluate triggers to identify potential clients suitable for effective case management intervention.
* Promote effective communications between all team members, client, family, provider and other parties involved in the case.
* Ensure the clients best interests are represented in the evaluation, treatment and management of their health care needs.
* Develop a healthcare plan that addresses the immediate, short-term and ongoing needs as well as where and how client's care needs to be met.
* Routinely make comprehensive and independent assessment of the client's status and progress toward reaching the goals set forth in the care plan.
* Performs other duties as assigned.

Qualifications:

Education:

* Bachelor's Degree in Nursing (BSN) is required.

Experience:

* Minimum two (2) years of experience in a health related setting or Community Based Organization.

Knowledge and Skills:

* Excellent relationship building skills.
* Excellent and effective written/verbal communication skills.
* Ability to effect change.
* Critical analysis.
* Excellent planning and organizing skills.
* Ability to promote client/family autonomy. 
* Knowledge of funding resources, services, and clinical standards and outcomes.
* Ability to fully utilize BHMC's electronic record systems, equipment and other healthcare and billing systems relevant to this position.

Licenses, Certifications:

* BLS Certification is required.

Physical Requirements:

* Position requires prolonged periods of standing, reaching, walking throughout the working day. 
* Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
