Conduct comprehensive assessments of patients' medical, social, and psychological needs * Collaborate with the treatment team to develop individualized care plans. * Coordinate and monitor the delivery of services, including therapy, medical care, and support services * Advocate for patients' rights and ensure their needs are met throughout their rehabilitation journey * Provide education and support to patients and their families Qualifications: What you will need: * Bachelor's or Master's degree in a related field preferred (e.g., Social Work, Nursing, Psychology) * Clinical case management experience, preferably in a rehabilitation or healthcare setting * Strong interpersonal and communication skills * Knowledge of brain injuries and rehabilitation preferred
