Job Summary

This position will provide case management services to families eligible to participate in the YMCA Child Placing Agency Program and Behavior Rehabilitation Services Program. This will include supporting YMCA licensed caregivers of youth in foster care in the King County, Snohomish County, and Peirce county areas, youth enrolled in intensive in-home stabilization services, and youth enrolled in therapeutic foster care services. Case management services include coordinating or providing services to enable the child and caregivers to utilize medical, educational, social, clinical, and other services necessary for maintaining the child's physical, psychological, and/or developmental health. You will also provide leadership and support to the greater Case Management team and ensure that all clients in the CPA and BRS programs are receiving quality case management services.

This position is at the Social Impact Center Y, the social services branch of YMCA of Greater Seattle (YGS). The mission of the Social Impact Center Y is to accelerate young people's ability to build safe, successful, and happy lives. Every year, we serve 5,000 children, youth, young adults, and families across King County with programs that aim to strengthen young people's ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; ensuring stability, safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Center Y's programs include housing, behavioral health, foster care licensing, violence prevention, education, and employment. Many of the young people we serve have had involvement in the foster care, criminal justice system, and/or homelessness systems.

Hiring Range: $24.00 - $27.00/hr

Responsibilities:

* Provide case management and care coordination services, as outlined in the CPA and BRS Contracts, Statements of Work, Handbooks, and Policy/Procedures, to clients enrolled in the CPA and BRS Programs including, licensed and unlicensed caregivers of youth in foster care, therapeutic foster care, intensive in-home stabilization services, and Outpatient Mental Health services. 
* Maintain a caseload size in accordance with contractual requirements and guidelines.
* Ensure clients receive all required screenings, assessments, services, interventions, and opportunities needed to fully meet their physical, psychological, social, emotional, relational, developmental, educational, independent-living, housing, and permanency needs in a timely and effective manner.
* In coordination with the Program Director, and in collaboration with the child & family team, ensure clients' transitions are well coordinated and facilitated, including placements & discharges, and ensure all required intake, service planning, and discharge/exit documentation is completed fully and on-time. Provide leadership and guidance in the placement referral process, and ensure that case transfers (of CMs, Intervention Specialists, and Child & Family Therapists) are smooth and as least disruptive as possible to the client.
* Ensure clients' parent/child/sibling visitation needs and requirements are met. Ensure natural supports and life-long connections are developed and stabilized.
* Coordinate and ensure inclusion of the client's Child/Family Team in the planning and decision-making process.
* Ensure foster parents/caregivers receive the necessary support services (case-management, care coordination, respite, resources, etc) in order to fully meet the client's needs.
* Ensure all services provided are well documented in the client's file in accordance with all WAC and contractual requirements and guidelines.
* Ensure all contract required client progress reviews and reports are completed thoroughly and accurately, are well written, and submitted on-time.
* Ensure client Referral Agreements and Exceptional Cost Plans are developed to fully represent the client's needs; in accordance with policy/procedure; receive required approvals; and are submitted and renewed on-time.
* Ensure each client's file/record is fully in compliance with WAC, DSHS/DLR, DSHS/CA, DSHS/DBHR, KC-BHO, COA Accreditation, and YMCA requirements.
* Ensure all clients' information is kept confidential and only released in accordance with HIPAA, DSHS, and YMCA policy and procedure.
* Conduct Intake Assessment for clients.
* Create Individual Service Plans or Caregiver Support Plans for clients. Monitor progress and review plans on a quarterly basis.
* Participate in clinical consults for clients as necessary in order to be able to provide clinical advocacy for clients. In conjunction with health professional, assess clients' mental health needs and determine appropriate level of care and support.

Code of Conduct for Applicants 

Qualifications:

* Bachelor's degree in social work, social services, or a closely related field from an accredited school.
* Two or more years' experience in providing social services to youth and families, preferably in mental health and child welfare settings.
* Knowledge of crisis de-escalation.
* Ability to teach effective independence skills for a variety of learning styles and to plan supportive activities.
* Strong youth engagement and relationship-building skills.
* Strong verbal and written communication skills.
* Basic computer skills and working knowledge of Microsoft Office.
* Ability to transport self and client(s) in a safe and effective manner, and to meet clients at community locations; proof of adequate vehicle insurance coverage is required, or ability to travel to areas without public transportation.
* Current state approved First Aid and CPR certification.**
* Current approved HIV blood borne pathogens training.**
* Ability to pass a DCYF Criminal Background Clearance

** Within 30 days of employment or first available training.

Authorized Driver Requirement

This safety-sensitive role requires transporting clients as "authorized drivers" for the Y using your own vehicle. This offer is contingent upon meeting the following conditions: Being age 21+, having a valid driver's license for at least three years and possessing a driving record that meets Y standards (no more than two moving violations within the last three years, no significant incidents such as reckless driving, DUI/DWI, no tickets 20+ miles above the speed limit etc. within 5 years and proof of current auto insurance, and current vehicle registration is required. (*Due to state-specific reporting requirements in our fleet management system, we are not currently able to enroll California Driver's License holders as Authorized Drivers) You must also pass a drug screen that will test for impairing substances, including marijuana.
