Responsibilities:

What You'll Do:

The Case Manager - CARES Co-Responder is responsible for helping those with mental health issues gain access to community services. Duties include meeting with individuals in crisis and their families in the community and connecting clients with professional services, crisis, and outpatient as needed. The CARES Co-Responder works collaboratively with the Multidisciplinary (LE) Treatment Team to respond to crisis situations and coordinate a plan of care utilizing expertise in counseling and crisis intervention, to ensure optimization of mental health outcomes for individuals and their families. The primary focus of this role is to connect individuals in crisis with resources and services to assist them in stabilizing their homelessness, mental health, and primary health situations.

* Intervene in community crisis situations with patients and families in the community, and with Law Enforcement.
* Assess, document, and address psychosocial, cultural, emotional, and economic needs for individuals and families during their immediate need/crisis situation.
* Collaborate with the Multidisciplinary treatment team, outpatient care team, and community agencies as needed to creatively address and or resolve the crisis situation.
* Identify and address gaps in service needs for participating parties' in-service delivery to the individual.
* Serve as an educational resource for potential clients/individuals in crisis.
* Encourage and support from a place of experience and understanding and willing to use appropriate self-disclosure.
* Demonstrates knowledge of community resources to help meet individual's unique recovery needs. Implements knowledge of multiple paths of recovery and promotes them equally.
* Collaborate with Interdisciplinary treatment team to engage, monitor, and communicate with the individual ensuring that safety is maintained while meeting the individual's needs.
* Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individual regarding treatment, as necessary. 
* Document all services and activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, etc. Ensure documentation in the Electronic Health Record (EHR) is clear, concise, and organized.
* Maintain a working knowledge of the Title 36 Involuntary Commitment process. Monitor the petition process to ensure timeliness is adhered to, and forms are complete and thorough, and when subpoenaed, attend COE hearings.
* Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency.  Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.
* Assist individuals and families in making difficult decisions, while honoring their preferences and values, in order to move the discharge plan forward.
* Performs all other duties as assigned.

Qualifications:

What You'll Bring:

* A Bachelor's degree in Behavioral Health or related field and at least 1 year of related experience, OR 
* An appropriate combination of education and experience
* Experience in a community based or crisis healthcare setting
* CPR/First Aid certified
* Valid driver's license
* Successfully pass a motor vehicle record check and maintain a clean driving record
* Must have own vehicle that is clean, neat, and in good working condition
* Willing to obtain Full coverage car insurance with high-level liability
* Arizona Level I Fingerprint Clearance Card
* The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company's exemption process prior to their start date as a condition of employment

It would be great if you had:

* 2 years of Behavioral Health Experience in a community based or crisis healthcare setting
* Knowledge of evidence-based/person-centered skillsets, such as motivational interviewing, trauma informed care, verbal de-escalation, harm reduction, and person-centered language
* Bilingual in Spanish
* Experience using EHR
