As part of the interdisciplinary care team, provides broad range of social work care management services to identified high risk patients. Supports patients and families by providing care management during transitions of care. Assists Primary Care Physicians in assessing need and utilization of home health services.

Qualifications:

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

Experience Requirements: 

* Three to five years care management experience or a combination of relevant experience
* Experience in health care or community setting working with geriatrics, i.e. case management, home health care, discharge planning or department of social services 
* Experience with Care Transitions coaching model, Motivational Interviewing and managing psych/social issues with patients and families
* Knowledge of case management, community resources/agencies, NCQA, QA, Medicare/Medicaid guidelines, program and workflow development, process improvement and implementation.

Education Requirements: 

* MSW or Master's Degree in related fields such as counseling, gerontology etc.

License/Certification: 

* LSW or LCSW Preferred
* Certification in Care Management preferred
* Current BLS certification required from American Heart Association required

Physical Requirements:
Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally
