Located on a beautiful 10-acre campus in the heart of Scottsdale, Devereux Arizona's 24-hour Residential Treatment Center (RTC) has the capacity to serve 52 children. The RTC is a safe, supportive environment that provides various services to youth, from the ages of 7 to 17, who are experiencing emotional, behavioral, or substance abuse challenges. The Campus features dormitories, a therapeutic K-12 day school, food services, medical/nursing, and therapy services. Reporting to the Clinical Program Manager, the Care Coordinator works cooperatively with clinical, operations, medical, and administrative staff on the overall coordination of individual care at the RTC. The Care Coordinator participates in family engagement/outreach, admission/intake processes, discharge planning, treatment planning, and documentation of all care coordination activities accordingly. EOE Qualifications: * Must be 21 years of age or older * HS Diploma or GED required; Bachelor's degree in Human Services, Psychology, Social Work or relevant field preferred * Four (4) years' experience in mental health/social services field with high school diploma/GED OR one (1) year of related experience with Bachelor's degree * Must demonstrate excellent verbal and written communication skills * Ability to handle multiple tasks and meet deadlines with minimum supervision; self-motivated * General knowledge of clinical best practices for working with adolescent populations and their families/resources * General knowledge of mental health, funding and related services within Arizona and other states served. * General knowledge of standard office computer applications. OTHER: * Must be available to attend 2 weeks of required orientation training (paid, Mon-Fri 8:30am-5pm) plus additional 2-3 weeks on the job training * Drug test within 24hrs of offer w/ negative results * Pre-employment Physical and Job Demand Assessment * Valid Arizona Driver's License & pass DMV 36-month history check * Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions). Arizona Revised Statutes Notice (ARS 36-411 Residential care institutions; nursing care institutions; home health agencies; fingerprinting requirements; exemptions; definitions).
