Responsibilities:
The Care Coordinator Assistant provides clerical, secretarial and administrative support for the Care Coordination department. Under the direction and supervision of the licensed staff, the Care Coordination Assistant performs the following activites and processes: post acute care coordination (SHF, HHC, DME, Hospice, etc.,) revenue related activities ( authorizations, certifications and payer communications ), stocking and monitoring office supplies and equipment and maintaining accurate and complete departmental records. This position may function as the point of contact for the department, and is responsible for the professional and timely response to calls, messages and the forwarding of communications to the responsible party. The Care Coordinator Assistant may be assigned other duties within the scope of the role, including, but not limited to, scheduling, faxing, auditing ,transcribing, filing, delivering, sorting, editing, tracking and correlating the departments documentation, records and forms.This position's emphasis will be on optimizing the licensed staffs productivity by providing clerical support, managing confidential patient information and ensuring the compliant and accurate management of departmental records and communications. The Care Coordination Assistant performs these duties according to appliciable hospital policies, standards of practice and Federal and State regulations.

Essential Job Functions:
* The Care Coordinator Assistant complies witih confidentiality policies, HIPPA regulations and organizational standards when transmitting protected patient information to external agencies and vendors as needed for placement, payment or referral.
* The Care Coordinator Assistant provides administrative support for the department by peforming assigned tasks or activities that do not require a clinical license.
* The Care Coordinator Assistant ensures office supplies and department equipment are available, current and functional.
* The Care Coordinaton Assistant schedules arranges and facilitates post acute care referrals, DME or follow up appointments under the supervision of licensed staff. This includes daily updates or communication between the department and post acute provider regarding changes in anticipated discharge date, needs or plan.
* The Care Coordination Assistant prioritizes and forwards communications relating to insurance requests, denials and Physician Advisor status recommendations to the appropriate Care Coordinator and documents any related authorizations,denials or status determinations in the designated EMR or software program.
* This position may obtain patient or family signatures on required documentation under the supervision of the clinical staff and maintains such documents in accordance with hospital policy.

* The Care Coordinator Assistant is accountable for staying current with and seeking knowledge of hospital policies, standards of practice and Federal or State regulations pertaining to their role.
Additional Job Functions:
* In addition, this position considers the population served by the medical center and area clinical integration programs and coordinates efforts to optimize care coordination across the care continuum. This coordination ensures a plan of care for patients in all stages of health needs.
Qualifications:
* Minimum of one year of experience supporting clinicians in demanding and fast paced patient care environments, and/or medical office experience.
* Current-Demonstrated experience in Microsoft Office, typing, and computer data entry.
* Proficiency in Microsoft Office applications including Word, basic Excel spreadsheet manipulation, typing, and data entry skills.
Education
* High School Graduate or equivalent education
* Associate Degree or High School Graduate with a Medical Office Certificate from an accredited institution.
* LVN preferred
Special Knowledge, Skills, Abilities
* Excellent customer service and presentation skills are a must
* Strong interpersonal and written communication skills are essential
* Demonstrated ability to apply analytical and problem solving skills
* Demonstrated ability to manage multiple tasks or projects effectively
* Ability to work independently as needed with a high degree of detail orientation.
* Ability to work efficiently in a fast-paced environment with changing priorities
