Why This Role is Special
As an RN Care Advisor, you will be pivotal in ensuring patients receive coordinated, high-quality care throughout their healthcare journey. You'll leverage your nursing expertise to conduct comprehensive assessments, develop patient-centered goals and care plans, and provide essential education and advocacy. By coordinating and managing care, you'll help patients achieve optimal health outcomes and enhance their overall healthcare experience.
What you can expect:
* Works with the patient, physician and care team, to create and implement a care plan that addresses the identified needs.
* Will serve as the primary contact, advocate and resource for patients, families and physicians.
* Assesses patient's knowledge of their clinical condition(s) and provide education and self-management support based on the patient's unique learning style.
* Will measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.
* Performs ongoing education to patient and patient's family and/or care giver on diagnosis, prognosis, treatment and medications.
* Serves as advocate when problems and issues arise.
* Perform other duties and responsibilities as assigned.
Team Collaboration
You'll join a dynamic, interdisciplinary team of physicians, RN's, Social Workers, Respiratory Therapists, Dieticians, Pharmacists, and Outreach Coordinators. Together, you'll conduct comprehensive assessments of patients' health needs and develop individualized care plans. This collaborative approach ensures that patient needs are met holistically, leading to improved outcomes and a higher quality of care.
Schedule and Work Environment
* Schedule: Monday - Friday, 8am - 5pm
* Work Environment: Hybrid (50% on-site / 50% remote)
* Location: Boise Plaza 4, Boise Metro area
This role offers a high degree of independence after training, with a supportive team to help you achieve patient goals. You'll manage and organize your day, including quality audits and chart reviews.
Qualifications:
* Education: Bachelor's of Nursing Degree (BSN)
* Experience: 2 years' RN experience- (Preferred experience listed below)
* Licenses/Certifications: Current RN License in the State of Practice
