Dignity Health Medical Group (DHMG) employs approximately 350 providers and 850 support staff who cover a wide variety of specialties. The medical group has had tremendous success over the past few years and now provides more than 73 sub-specialty services. The physicians and APPs provide clinical services in their areas of specialty and many serve in pivotal academic research and leadership roles. Join our dynamic and growing cardiovascular team, where you'll play a pivotal role in providing exceptional care with a focus on women's heart health and prevention. We are seeking a highly motivated and compassionate Heart Failure Cardiovascular Advanced Practitioner to join our team in Phoenix, Arizona. You will play a crucial role in providing comprehensive care to patients with cardiovascular conditions, including but not limited to the following.
Some details:
* Will join the Heart Failure and LVAD team. Currently two doctors and 3 NP's. 1 NP is fully supporting HF, 1 NP fully supports LVAD and 1 is split between both services.
* Mix of patient volumes but high cases of medicaid, uninsured, underinsured.
* Approximately 10 patients on list per day in hospital and about 5-7 in clinic when seeing patients in the office.
* Clinic 2 days per week.
* Daily inpatient and consults.
* Work week will vary depending on assignment of Inpatient vs Outpatient. The usual scheduled day starts at 7:30 am and is until about 5pm but can be dependent on the attending physician.
* Procedures - CardioMems monitoring, and various monitoring devices specific to Heart Failure.
* Call - Required 1 day per week and 1 weekend per month. Anything extra is compensated with additional pay.
* Developing service line.
Responsibilities:
* Provide comprehensive cardiovascular care.
* Perform patient evaluations, diagnostic assessments, and manage follow-up care, ensuring continuity and high-quality outcomes.
* Collaborate with your supervising physician and a multidisciplinary team to develop and implement personalized treatment plans.
* Participate in community outreach, patient education, and cardiovascular research.
* Order, interpret and analyze diagnostic tests.
* Prescribe and administer medications and physical therapy as necessary.
* Participants in quality assurance and quality improvement for patient care.
* Works within a multidisciplinary team.
* Additional duties as assigned by leadership.
Qualifications:
* Master's Degree in Nursing (MSN) or Physician Studies
* Graduate of accredited program
* Arizona Advanced Practice license
* Must hold and maintain National Certificate as an NP/PA in area of specialty under AZ regulations
* Valid DEA registration
* RNFA or in process
* Must meet credentialing criteria for medical staff
