Presbyterian is hiring a Bereavement Coordinator to support Hospice. 

This position is responsible for assessing developing and implementing appropriate bereavement care plans that support the emotional, psycho-social and spiritual support and services before and after death of the patient and to assist with issues related to grief, loss and adjusting.

Responsibilities:

* Provides post-death bereavement services to hospice families
* Provides grief support to community bereaved
* Participates in the bereavement program development
* Provides education and advocacy about grief, loss and bereavement to both hospice staff and the community
* Develops and distributes educational materials about grief, loss, and bereavement.
* Demonstrates understanding of definitions and principles of bereavement, loss, and grief
* Address high-risk needs.
* Assures timely ongoing assessment of individual bereavement needs.
* Appropriately assess bereavement needs and evaluates the effectiveness of interventions, adapting the Plan of Care accordingly.
* Demonstrates timely, accurate, and appropriate documentation of bereavement contacts.
* Participates in IDT-Interdisciplinary Team
* Participates in other hospice clinical, operational committees

Qualifications:

* Masters Degree in counseling, social work, psychology or other professional with documented evidence of training and experience in dealing with grief required.
* 3 yrs previous Hospital or Hospice experience required. 

Education Essential: Master Degree
