				Overview:

* Provide comprehensive Behavior Health services to support residents' full potential during their stay at the Nursing Home.
* Coordinate care delivery and conduct reviews of services to determine appropriate level of care utilizing standardized testing measures as well as safe, clinically appropriate care.
* Provide pre-admission and concurrent reviews for Behavioral Health levels of care and coordinate daily administrative and clinical operations.

Responsibilities:

* Work to ensure linkage and integration of behavioral health principles into the medical care provided to each resident.
* Provide timely and accurate completion of resident assessment information and other forms as required in accordance with federal, state and local regulations and facility/company policies and procedures.
* Work closely with the facility mental health providers to insure all resident psychosocial/psychiatric needs are identified; referrals made; and services are provided.
* Provide direct consultation to facility staff with care management and regulatory standards in Behavioral Health.
* Participate in care planning by supporting emotional and psychological needs of the residents.
* Conduct utilization reviews, providing and summarizing clinical information to supervisors and physicians to ensure appropriate medical care coordination and service delivery.
* Provide continual education to facility staff in Behavioral Health through hands on demonstration during normal care delivery routines.
* Participate in Referral Management of all potential Admissions with Behavioral Health concerns including but not limited to on-site reviews.

Qualifications:

* Registered Nurse or Licensed Practical Nurse in good standing with state licensing board, or master's degree specializing in Social Work and/or Licensed Social Worker with training in behavior, dementia and/or psychiatric care.
* Recommend (2) years' experience in behavioral management, mental health and/or long-term care.
* Currently hold Crisis Prevention Institute certification or willing to achieve within first 3 months.
