Qualifications

* Current license and certification as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC).
* Proven experience in behavioral health integration within primary care settings.
* Expertise in behavioral and cognitive behavioral interventions.
* Strong consultative and collaborative skills.
* Exceptional communication, organizational, and interpersonal abilities.

Preferred Attributes

* Experience in chronic disease management, psychoeducation, and risk assessment.
* Familiarity with program development and patient education material creation.
* Ability to foster collaboration between PCPs and psychiatrists.

Essential Duties and Responsibilities The essential duties and responsibilities of this position include the following and must be completed with or without reasonable accommodation:

Behavioral Health Integration

* Operate in a consultative role within the primary care team using the Primary Care Behavioral Health Model.
* Provide recommendations regarding behavioral interventions to referring PCPs.
* Conduct brief interventions with referred patients on behalf of the referring PCP.
* Maintain a visible presence to the care team during clinic operating hours to support collaboration and consultation.

Clinical Excellence

* Perform same-day and scheduled initial consultations with patients.
* Conduct brief, limited follow-up visits for selected patients.
* Provide a range of services, including screening for common conditions, assessments, and interventions related to chronic disease management programs.
* Conduct risk assessments as needed.
* Deliver psychoeducation to patients during individual and group visits.
* Provide brief behavioral and cognitive behavioral interventions for patients.
* Triage patients with severe or high-risk behavioral problems to community resources for specialty mental health services.
* Facilitate referrals to specialty mental health services and support smooth transitions from specialty care back to primary care when appropriate.

Program Development and Education

* Assist in the development of clinical pathway programs, group medical appointments, classes, and behavior-focused practice protocols.
* Maintain an up-to-date library of patient education materials tailored for literacy levels and cultural appropriateness.
* Identify, review, and modify educational materials to enhance accessibility.
* Educate PCPs in the principles of brief behavioral and cognitive behavioral interventions and reinforce their use in medical visits.

Documentation and Collaboration

* Prepare brief consultant notes for the medical chart summarizing assessment findings, interventions delivered, and recommendations made to the PCP.
* Maintain clinical records and complete necessary paperwork in a timely manner to comply with administrative regulations.
* Collaborate with PCPs and psychiatrists concerning medication protocols and treatment plans.
* Provide assistance in capturing program evaluation and fidelity measures.

Meetings

* Attend and contribute to clinic meetings.
* Participate in monthly meetings with the Population Health Team and Signal Lamp stakeholders.
* Lead weekly Interdisciplinary Team (IDT) meetings.
