				Overview:

The Behavioral Health Care Specialist works with patients to assess and determine needs related to therapeutic interventions and appropriate follow up care, implements treatment plans, coordinates the treatment process and discharge planning, and facilitates groups related to maintaining wellness upon discharge. Also demonstrates the knowledge and skills necessary to provide services appropriate to the age and cultural diversity of the patients served. In addition, the Behavioral Health Care Specialist will uphold and demonstrate the Novant Mission, Vision and Values: Compassion, Diversity, Personal Excellence, and Team Work.

Responsibilities:

Essential Functions
   
Implements individualized treatment plans using best practice modalities and interventions to obtain optimal patient outcomes. Delivers on-site care to a caseload of Behavioral Health patients in the acute care setting. Provides services including 1:1 brief solution focused interventions, psycho-educational groups, and conducting family meetings as needed. Demonstrates proficiency in working with individuals of various ages, diagnoses, and levels of acuity requiring a foundation of knowledge in both substance use and mental health disorders. Works collaboratively with each patient and an interdisciplinary team to identify current treatment as well as aftercare needs and execute the safe discharge plan established based on patient needs and available resources. Obtains and Utilizes knowledge of various levels of care and current community resources in order to connect each patient with appropriate support in the community to best promote ongoing recovery. Secures aftercare appointments, verifies living arrangements, and facilitates placement in other long-term facilities as needed. Complies with required standards for timely and through documentation as well as for medical/legal reporting situations. Assists with Involuntary/Voluntary commitment process when indicated and assures that treatment provided is in accordance with EMTALA regulations.

Other Duties: Participates in program development, implementation and evaluation. Interacts and communicates with community agencies providing liaison and referral services. Continues professional development and provides documentation to be kept in employee's personnel file. Contributes to the effective functioning of the Behavioral Health programs, as well as facility-wide activities and services. Guidance/direction/problem solving is guided by best practice and management review. Knowledge and compliance with all regulatory standards such as the Joint Commission and CMS. May require knowledge of Utilization Review practices and processes. Assists with admission process when indicated. Participates on the Psychiatric Rapid Response Team to assist in de-escalation of patients in other areas of the hospital during behavioral health crises (excluded hospital based IOP/PHP Behavioral Health Care Specialists). Establishes and maintains knowledge of processes such as petitioning for guardianship and navigating the Medicare appeals process. Willingness to cross train between Inpatient and Emergency Departments to assist during periods of need.
 
Confidentiality: Use and analyze patient history assessments and/or clinical data. Provide and receive input about patient billing, finances, and/or insurances. Access corporate business strategy. Adheres to Novant Health policies and procedures related to patient confidentiality, information sharing and patient rights.

Communication and Documentation: Communicates individualized plan of care and patient's progress toward goals to all members of the care team including the patient and his/her chosen support system/legal guardian/Care Coordinator. Consistently utilizes appropriate communication within the interdisciplinary team and when handing off patient care. Escalates complex cases and engages in complex case meetings involving outside departments for guidance in patient care and discharge coordination. Documents in a timely manner in accordance with Novant Health's documentation policies and procedures. Accepts direction and feedback from leaders and peers.

Teamwork: Develops and maintains positive relationships with team members, leadership and community providers. Consistently demonstrates a willingness to adjust activities accommodating the needs of team members. Develops team members through mentoring, precepting, orientation and feedback. Resolves conflicts independently. Accepts responsibility for actions and responds positively from feedback. Supports team members related to occupational stress.

Behavioral Health: Knowledge psychiatric medications, psychiatric diagnosis, mental health concepts, and evidence based therapeutic interventions and treatments. Ability to establish and maintain control of violent patients by utilizing verbal de-escalation skills, defend self and others against violent assaults, ability to react appropriately during a psychiatric crisis. Ability to intervene in a therapeutic manner for behavioral patterns demonstrated by patients with a mental illness, personality disorder, abusing substances or developmental disabilities.

Qualifications:

* Education: 4 Year / Bachelors Degree, required. Degree in social work, psychology, human services or similar health related field. 
* Experience: Minimum of one year experience, preferred. 
* Licensure/Certification: NC state approved curricula for restrictive interventions (facility specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required. Resuscitation Quality Improvement, required. Notary Public, preferred. 
* Additional Skills (required): Good working knowledge of computers. Ability to care for Behavioral Health Age Specific populations.
* Work schedule: PRN 7pm - 7am
