Hello humankindness Dignity Health Medical Group is the employed physician group of Dignity Health Arizona. Dignity Health Medical Group (DHMG) employs approximately 400 providers and 1200 support staff that cover a wide variety of specialties. The medical group has had tremendous success over the past few years and now provides more than 73 subspecialty services. The physicians provide clinical services in their areas of specialty and many serve in pivotal academic, research and leadership roles. DHMG is also heavily involved in preparing tomorrow's healthcare providers. DHMG has 84 medical school students and approximately 200 residents and fellows throughout the 25 academic programs. Clinical services are complemented with translational and bench research to augment medical education for residents and students. Responsibilities:

The Behavioral Health Care Manager is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.

Qualifications:

MINIMUM

Master's degree in either Counseling or Social Work

2 + years experience as a mental health counselor in a behavioral health setting

Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Work (LCSW), Licensed Masters of Social Work (LMSW) 

American Heart Association BLS

PREFERRED

Licensed Marriage and Family Therapist OR Licensed Social Worker OR Nurse Practitioner OR Licensed Psychologist

Experience providing brief psychosocial interventions such as problem-solving treatment

Experience with evidence-based counseling techniques
