Overview:
Novant Health is seeking a Behavioral Health Care Specialist for our therapy team. The Behavioral Health Care Specialist works with patients to assess and determine needs related to therapeutic interventions and appropriate follow up care, implements treatment plans, coordinates the treatment process and discharge planning, and facilitates groups related to maintaining wellness upon discharge. Also demonstrates the knowledge and skills necessary to provide services appropriate to the age and cultural diversity of the patients served. In addition, the Behavioral Health Care Specialist will uphold and demonstrate the Novant Mission, Vision and Values: Compassion, Diversity, Personal Excellence, and Team Work.
The care specialist works to ensure safety, best practice and high-quality standards of care maintained throughout the discharge planning process and establishes care connections with providers when patients leave the hospital. They collaborate with providers and all members of the interdisciplinary treatment team to assess, plan, implement and evaluate proactive complex and safe discharge plans with patient involvement. The care specialist serves as a resource for discharge planning and care coordination, requiring familiarity with local community resources and various levels of Behavioral Health Care. They must demonstrate the knowledge and skills necessary to provide services appropriate to a patient population comprised of various ages (18+), levels of acuity, and mental health and/or substance use diagnoses. Previous experience in the hospital setting or with crisis response would be highly valuable in this role.

Qualifications:
* Education: 4 Year / Bachelors. Degree in social work, psychology, human services or similar health related field, required.
* Experience: Minimum of one year experience, preferred.
* Licensure/Certification: NC state approved curricula for restrictive interventions (facility specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required. Resuscitation Quality Improvement, required. Notary Public, preferred.
* Additional Skills required: Good working knowledge of computers. Ability to care for Behavioral Health Age Specific populations.
