St. Luke's is seeking an Ambulatory Care Pharmacist to join our team. This position is embedded within a large primary care clinic in McCall, Idaho. This role is vital part of the health care team within our clinics!
The St. Luke's Ambulatory Pharmacy Department includes pharmacists embedded in clinics across the health care system within the following specialties: internal medicine, family medicine, endocrinology, anticoagulation, heart failure, lipidology, cystic fibrosis, and children's specialties.
What you can expect from this role:
* An independent clinic-based pharmacist practice with broad collaborative practice agreement
* Management of a fast-paced clinic schedule of 8-12 patients appointments per day
* Providing support to providers and patients through answering drug information questions, trouble-shooting pharmacy problems and educating on medications
* Working as part of a multidisciplinary team with physicians, advanced practice practitioners, behavior health specialists and care managers
* Collaboration with clinical pharmacy technicians
* Opportunities for precepting pharmacy students and residents
Preferred Qualifications for this Role:
* Previous experience with the clinical management of primary care based chronic diseases.
* Completion of PGY2 Ambulatory Care OR completion of a PGY1 pharmacy residency program with ambulatory care-focus OR equivalent ambulatory care experience.
* Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP).

Responsibilities:
The Pharmacist is responsible for the delivery of pharmaceutical care to patients and customers of the medical center.
* Review appropriateness of prescribed medication therapy.
* Provide medication information and consultation to patients, nurses and medical staff.
* Interpret prescriptions, compound and dispense medications.
* Supervise and authorize the work of technicians, externs, interns and support staff.
* Prepare and maintains all necessary records on medication therapy, prescriptions, patient charges, medical records and inventory.
* Perform cognitive clinical pharmacy services and document findings and actions in accordance with standards of the medical center.
* Administer medications as appropriate.
* Administer vaccinations as appropriate.
* May assist in the training of technicians, externs, interns, residents and other department staff.
* Serve as a role model and mentor to technicians, students, residents and new practitioners.
* Perform other duties and responsibilities as assigned.
Qualifications:
* Education: Degree from an accredited pharmacy program
* Experience: 0 years' experience
* Licenses/Certifications: Valid Pharmacist License in state of practice AND Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
