Overview:
POSITION SUMMARY:
The Adult Pallative Care Counselor provides counseling, critical psycho-social support to palliative care patients through the palliative Care Service consult team. Responsible for the provision of psychological services to patient and their family affected by chronic and/or life-limiting conditions. Conducts appropriate assessment of patient and family mental health needs. Provides consultation to healthcare professionals regarding the impact of psycho-social factors on patients' illness.

REQUIRED QUALIFICATIONS:
Education:
Masters
Licenses/Certifications:
LPC-Licensed Professional Counselor or LSW - Licensed Social Worker
Experience:
3 years related work experience in social work or counseling
Required Other skills/knowledge:
Excellent interpersonal and communication skills.
PREFERRED QUALIFICATIONS:
LCSW - Licensed Clinical Social Worker or LCPC - Licensed Clinical Professional Counselor
